On this page members can complete forms to request charitable contributions to their favorite charity. This is a one time request that must be resubmitted each year. 

Members can also complete an Educational Assistance Request (Scholarship) for dependent children.

RSVP For Dinner/Special Events

Use this link https://forms.office.com/r/8xizFMpy6C?origin=lprLink or the QRCODE on the right to make a reservation for dinner before our meetings or for a Special Event . Be sure to select the proper meeting from the dropdown.

Charity Request Form

The Charity request form should be completed and submitted to charity@lodge126.com. Most non-masonic charity requests are approved at the end of the year (November or December).  The deadline for end of year donation requests is October 31.  If a donation is time sensitive for a specific event the form should be submitted 15 days before the stated meeting prior to the event.

Scholarship Request Form

The Lodge offers Scholarships to dependents of members who meet the following requirements

  • Member is a Master Mason in good standing with 3 years of service
  • Applicant is attending an accredited college/university in the United States
  • Applicant has a 3.0 or greater GPA from the previous year (High School or College)
  • The deadline for this form to be submitted for the Fall Semester is May 31. 

After this requested is submitted the member must provide the following documentation as soon as it is available

  • A copy of member's tax return showing that the applicant is a dependent (numbers may be blacked out)
  • A copy of applicant's transcript from school showing GPA
  • An invoice from the school including the student id & the pay to information. 

If the scholarship is granted, a check will be made out jointly to the applicant and the school.
